Chase and Morris counties are partnering on a community health assessment.
Residents in both counties are asked to take part in a short online survey designed to gauge community health needs, including use of available health services, perceptions of local health services and potential improvements or additions. The results are of interest to the Chase and Morris County Health Departments, Morris County Hospital, USD 284 Chase County, USD 417 Morris County and USD 481 Rural Vista.
Wichita State’s Community Engagement Institute will combine the survey responses into one report. Responses will be kept confidential. If you have questions, email tara.gregory@wichita.edu or call 316-879-3714.
