Newman Regional Health Medical Partners is being noted by the Kansas Healthcare Collaborative for its work in population health and chronic disease management.
The Healthcare Collaborative says Newman Medical Partners is one of 11 medical practice groups in the top 25 percent of baseline assessments. The assessment focused on five areas for improving health outcomes by minimizing unnecessary hospital stays, making care delivery more efficient and by creating cost savings, including:
*Enhancing the use of electronic records data
*Collaborating with the community
*Strengthening a team-based care approach
*Implementing self-measured blood pressure monitoring
*Reinforcing diabetes prevention efforts
The assessment process is a joint quality effort involving the Healthcare Collaborative and the Kansas Department of Health and Environment’s Bureau of Health Promotion.
